Health Information
2024-25 Physical Examinations for Students
In accordance with New York State regulations, student physical examinations are required for all students in grades K, 1,3, 5, 7, 9 and 11 and also for all students new to the Pearl River School District.
While we continue to recommend annual physical and dental examinations by your family physician and dentist, only the parents of new students and grades K, 1, 3, 5, 7, 9 and 11 should download the appropriate required physical and dental forms (listed at right). Physical examinations must be done within 12 months of school entry in order to comply with State requirements for the upcoming school year.
A physical dated after September 3, 2023 will fulfill NYS requirements. Students who have not had a physical examination or are under immunized and/or do not have appropriate documentation on file in the health services office, will not be able to continue to attend school.
Required Documentation
Students in any grade who have not yet filed proof of required immunizations must also comply with state regulations. Pearl River School District also requires proof of a PPD (tuberculin test) in the year prior to entry to the school district, or a note from the physician stating it is not necessary. Immunizations
Each student registering for school is required to submit proof of immunization against:
- Measles, Mumps & Rubella
- Diptheria, Tetanus & Pertussis
- Poliomyolitis
- Hepatitis B
- Meningococcal Meningitis
- Varicella (Chickenpox)
- Haemophilus Influenzae
- Polio
Proof of a TB test within the past year is required. This documentation is the responsibility of the parent or guardian. Each year, the district is required to submit summary data regarding student immunizations. This data does not include identifying student information.
Physical Examinations
Each student entering school for the first time is required to submit proof of physical examination. Documents must be dated no more than 12 months prior to the start of the school year in which they are required, or the date of entrance to the school for new entrants. Additionally, students in the following grades are required to have a physical examination:
Kindergarten - Grade 1 - Grade 3 - Grade 5 - Grade 7 - Grade 9 - Grade 11
This exam may be performed by either the school physician or preferably by the child's personal doctor. The documentation is the responsibility of the parent or guardian. Parents are asked to submit an emergency action plan (reviewed by child's doctor) for students identified with severe allergies.

The district is required to submit summary data regarding body mass index (BMI). This data does not include identifying student information. Please contact your child's building nurse if you do not wish his/her individual data be reported.
Sports Physical Examinations
All students who participate in interscholastic athletics are required to have a physical examination prior to participation. This exam may be performed by either your family physician or the school physician (prior to the season opening), as indicated by the parent/guardian on the athletic permission slip.
Health History Update
Parents/guardians will receive a health history update to be returned annually in September.

Health Screenings
Each student will participate in health screenings as outlined by New York State School Health Services. These include vision, hearing, scoliosis, and physical examinations.
Administering Student Medications
All medication may be administered to students in school only through a doctor's note and parent's written request. Dosage and frequency of its administration, as well as diagnosis of the student's condition, must be indicated in the note. Medication must be in the original prescription bottle or packaging and must be given directly to the school nurse by the parents. Students may not carry any medication into school at any time. In order to keep our schools and students healthy, children who have been sick cannot attend school until they are no longer contagious: either 24 hours without a fever or 24 hours after they have been on an antiobiotic for an infection. Attending School After Being Sick
In order to keep our schools and students healthy, children who have been sick with a non-COVID illness cannot attend school until they are no longer contagious: either 24 hours without a fever or 24 hours after they have been on an antibiotic for an infection. Children who have been diagnosed with COVID-19 must follow the current CDC Guidelines at https://www.covid.gov/.
When a Student Becomes Sick at School
When a child becomes sick, or is injured at school, the home is contacted. If the parent cannot be reached, a neighbor or relative as noted on the emergency form is then called. In the event that the child requires immediate doctor's care or hospitalization and the parents cannot be reached, the school personnel then act in loco parenti and contact the child's physician or go to the nearest hospital. For these reasons, it is very important for the school to have the name and telephone number of your child's doctor and two neighbors.
Insurance
Student accident insurance is provided on a limited basis for all Pearl River students. The excess coverage policy insures all school activities, but insurance payments must be paid through private family policies first. Remaining medical bills are then covered according to a limited fee schedule. The district sends home policy information each year; additional information may be obtained from the nurse at each school.
Working Papers
Students seeking employment are required to provide a Physical Fitness Certification when applying for 'working papers'.

Asbestos
Since 1986, PRSD has kept a management plan, approved by the NYS Education Department, on asbestos. Every three years, district buildings are re-inspected and the plan updated. Every six months, the district completes a surveillance program on asbestos-containing materials in our schools. PRSD meets and exceeds all environmental laws with regard to containing asbestos. Copies of the plan and program are available for review in the district offices.
Pesticides
While PRSD relies on strategies other than pesticides as its primary control of pests, the occasional use of pesticides is necessary. Only certified individuals are allowed to apply these materials and only when no people are in a building or on a school field. PRSD will notify anyone who desires 48 hours prior to applying pesticides. To receive notification, contact Mathew Kelleher, Director of Facilities, at 620-3864.
