Health Information

Each student registering for school is required to submit proof of immunization against:

• Measles, Mumps & Rubella
• Diptheria, Tetanus & Pertussis
• Poliomyolitis
• Hepatitis B
• Meningococcal Meningitis
• Varicella (Chickenpox)
• Haemophilus Influenzae 
• Polio

Proof of a TB test within the past year is required. This documentation is the responsibility of the parent or guardian.  Each year, the district is required to submit summary data regarding student immunizations. This data does not include identifying student information. 

Each student entering school for the first time is required to submit proof of physical examination.  Documents must be dated no more than 12 months prior to the start of the school year in which they are required, or the date of entrance to the school for new entrants.   Additionally, students in the following grades are required to have a physical examination: 

Kindergarten - Grade 1 - Grade 3 - Grade 5 - Grade 7 - Grade 9 - Grade 11

This exam may be performed by either the school physician or preferably by the child's personal doctor. The documentation is the responsibility of the parent or guardian. Parents are asked to submit an emergency action plan (reviewed by child's doctor) for students identified with severe allergies.

The district is required to submit summary data regarding body mass index (BMI). This data does not include identifying student information. Please contact your child's building nurse if you do not wish his/her individual data be reported. 

All students who participate in interscholastic athletics are required to have a physical examination prior to participation. This exam may be performed by either your family physician or the school physician (prior to the season opening), as indicated by the parent/guardian on the athletic permission slip.

Parents/guardians will receive a health history update to be returned annually in September.  

Anaphylaxis (Severe Allergic Reaction) Emergency Action Plan

Each student will participate in health screenings as outlined by New York State School Health Services.  These include vision, hearing, scoliosis, and physical examinations.

All medication may be administered to students in school only through a doctor's note and parent's written request. Dosage and frequency of its administration, as well as diagnosis of the student's condition, must be indicated in the note. Medication must be in the original prescription bottle or packaging and must be given directly to the school nurse by the parents. Students may not carry any medication into school at any time. In order to keep our schools and students healthy, children who have been sick cannot attend school until they are no longer contagious: either 24 hours without a fever or 24 hours after they have been on an antiobiotic for an infection.   Authorization for Administration of Medication

In order to keep our schools and students healthy, children who have been sick with a non-COVID illness cannot attend school until they are no longer contagious: either 24 hours without a fever or 24 hours after they have been on an antibiotic for an infection.   Children who have been diagnosed with COVID-19 must follow the current CDC Guidelines at https://www.covid.gov/.

When a child becomes sick, or is injured at school, the home is contacted.  If the parent cannot be reached, a neighbor or relative as noted on the emergency form is then called.  In the event that the child requires immediate doctor's care or hospitalization and the parents cannot be reached, the school personnel then act in loco parenti and contact the child's physician or go to the nearest hospital.  For these reasons, it is very important for the school to have the name and telephone number of your child's doctor and two neighbors.

Student accident insurance is provided on a limited basis for all Pearl River students. The excess coverage policy insures all school activities, but insurance payments must be paid through private family policies first. Remaining medical bills are then covered according to a limited fee schedule. The district sends home policy information each year; additional information may be obtained from the nurse at each school.
