Physical Examinations for Current Students

In accordance with New York State regulations, student physical examinations are required for all students in grades K, 1,3, 5, 7, 9 and 11 and also for all students new to the Pearl River School District.

While we continue to recommend annual physical and dental examinations by your family physician and dentist, only the parents of new students and grades K, 1, 3, 5, 7, 9 and 11 should download the appropriate required physical and dental forms (listed at right).  Physical examinations must be done within 12 months of school entry in order to comply with State requirements for the upcoming school year.

A physical dated after September 1, 2022 will fulfill NYS requirements.  Students who have not had a physical examination or are under immunized and/or do not have appropriate documentation on file in the health services office, will not be able to continue to attend school.

 

Required Documentation

Students in any grade who have not yet filed proof of required immunizations must also comply with state regulations. Pearl River School District also requires proof of a PPD (tuberculin test) in the year prior to entry to the school district, or a note from the physician stating it is not necessary.  Links to Required Forms:
